Out-of-Network Status & Insurance Disclosure
Zest Healthcare LLC is an independent, out-of-network provider of durable medical equipment (DME), based in New Jersey.
We:
Do not participate in any federal healthcare programs, including Medicare or Medicaid.
Are not contracted with any commercial health insurance networks.
We will make reasonable efforts to verify your commercial PPO insurance eligibility and submit claims on your behalf as a courtesy. However:
Coverage and reimbursement are not guaranteed.
If your insurer denies or fails to pay the claim in full, you are financially responsible for the entire cost of the equipment provided.
Submitting a claim does not ensure payment or coverage.

Return & Refund Policy
Our return policy allows eligible items to be returned within 30 days of delivery with prior authorization (RMA required).
We offer free shipping and provide a free return shipping label for approved returns. Items must be new, unused, and in original packaging with all components; certain hygiene-sensitive products cannot be returned if opened.
Refunds for insurance-billed orders will be sent to the insurer; refunded to customer portions will be refunded to customer. Refunds are typically processed within 30 days of return approval.
Restocking fees may apply for used or damaged items. Returns without authorization or beyond 30 days will not be accepted.

Medical Necessity & Prescription Requirement
Zest Healthcare provides DME products only:
Upon receipt of a valid prescription, and
Confirmation of medical necessity by a licensed healthcare provider.
We reserve the right to request supporting documentation before fulfilling any order.

Continuous Glucose Monitoring (CGM) Disclaimer
All Continuous Glucose Monitoring (CGM) systems and related resupply products are intended for use only as prescribed by licensed healthcare providers. Zest Healthcare LLC:

• Does not provide patient training,
• Does not offer setup or installation, and
• Does not interpret CGM data.

It is the sole responsibility of the prescribing provider to ensure proper clinical use, interpretation, and patient compliance with CGM devices.

Laboratory / CLIA Regulation Disclaimer
Zest Healthcare LLC is not a clinical laboratory and does not perform or facilitate diagnostic testing. Any products that may fall under CLIA (Clinical Laboratory Improvement Amendments) regulations must be ordered and used only by providers who are properly licensed and certified under applicable laws.
Zest Healthcare assumes no responsibility for misuse or improper handling of any CLIA-regulated items.
