Out-of-Network Disclaimer
Effective Date: May 30th, 2025.
Zest Healthcare LLC is an independent, out-of-network supplier of Durable Medical Equipment (DME). We are not enrolled in Medicare, Medicaid, or any other federal or state healthcare program, and we are not contracted with any commercial insurance company or managed care organization.
As an out-of-network provider, we may submit claims to your commercial insurance plan as a courtesy, but only if you authorize us to do so by submitting the Intake & Assignment of Benefits (AOB) Form on our website.
By completing the Intake & AOB Form, checking the agreement box, and entering your full name, you:
Confirm that this action constitutes your express authorization for Zest Healthcare LLC to submit insurance claims on your behalf and to receive direct reimbursement from your insurance carrier, where applicable;
Acknowledge that this authorization is legally binding and complies with federal and state laws governing electronic records and signatures;
Understand and agree to the financial terms outlined below.
Important Financial Disclosures
Submitting a claim does not guarantee payment. Coverage is determined solely by your insurance provider and plan terms.
You may be responsible for deductibles, co-insurance, non-covered items, or denied claims. If your insurance denies or underpays a claim, you agree to pay the full cost of the equipment provided.
You are financially responsible for all charges regardless of your insurer’s determination, and agree not to withhold payment due to any pending claim, denial, or appeal.
Prescription and Product Limitations
All DME must be prescribed by a licensed healthcare provider based on medical necessity.
Zest Healthcare LLC does not provide diagnosis, treatment, medical training, or patient monitoring services.
We will not contact your healthcare provider for clinical clarification or decision-making.
All orders are contingent upon:
Confirmation of medical necessity,
Product availability, and Any applicable insurance eligibility or coverage verifications.
Returns & Refunds
You can return eligible items within 30 days of delivery, but you must first contact us to get a Return Merchandise Authorization (RMA) and a free prepaid shipping label.
Items must be unused, in their original packaging, and include all manuals and accessories; some hygiene or safety items cannot be returned if opened or used. We will not accept returns without RMA, and a restocking fee may apply if the item is damaged, worn, or incomplete.
For orders billed to insurance, refunds go to your insurance company, and any overpayment (amount paid for deductible, co-pay, or co-insurance) will be refunded to you. Refunds are usually processed within 30 days after approval. Items marked “Delivered” cannot be refunded if lost after delivery.

You acknowledge and accept Zest Healthcare LLC’s out-of-network status;
You authorize us to submit claims to your insurer on your behalf based on your Intake & AOB Form submission;
You agree to the financial responsibility terms outlined in this disclaimer, regardless of insurance reimbursement outcomes.
